Reminder: Immunizations and health screenings required for school

New York State law requires that children attending pre-K through grade 12 receive all required doses of vaccines on the state’s recommended vaccination schedule in order to attend or remain in school.

Within 14 days of the first day of school or daycare, parents must show proof of their child’s up-to-date vaccinations, or provide a valid medical exemption from vaccination.

In order to attend or remain in school, children who are unvaccinated or overdue must receive at least the first dose of all required vaccines within the first 14 days of school. They also must plan to receive all follow-up doses at the ages and intervals listed in the recommended vaccination schedule.
Similarly, state requirements for health examinations and in-school health screenings have changed. The new regulations require health exams for all new students and those entering the following grades: K, 1, 3, 5, 7, 9 and 11.

A New York State licensed physician, physician’s assistant or nurse practitioner must perform the examination. A copy of the health examination must be provided to the school within 30 days from the start of the school year. If your child has an appointment for an exam during this school year that is after the first 30 days of school, please notify the Health & Safety office with the date.

Completed physicals must be dated no more than 12 months prior to the start of this school year. We suggest you make copies of the completed forms for your own records before sending them to the school Health & Safety office.

    The new screening requirements are as follows:

  • Vision and hearing for new entrants within six months of admission and grades K, 1, 3, 5, 7, and 11.
  • Scoliosis screening will be required for girls in grades 5 and 7 and for boys in grade 9.
